Neutrophil to Lymphocyte Ratio (NLR) = Absolute Neutrophil count / Absolute Lymphocyte count
Indicator of inflammation (infection) and physiologic stress (e.g., hypovolemic shock)
Physiologic stress level
– NLR < 3 : Normal
– NLR 3 ~ 6 : Borderline
– NLR 6 ~ 9 : Mild stress
– NLR 9 ~ 18 : Moderate stress
– NLR > 18 : Severe stress

Effect of physiologic stress on NLR :
NLR Pitfalls:
- NLR may increase with exogenous steroid use.
- Cell counts may vary in situations such as leukemia, chemotherapy, and G-CSF use.
- In HIV patients, baseline NLR may be increased.
